Subcision is a dermatological procedure used to treat atrophic scars, including those after acne lesions. Atrophic scars are depressed scars that appear on the skin when the subcutaneous layer of collagen and adipose tissue is destroyed or damaged. This type of scar most commonly occurs following severe forms of acne or in patients who traumatize their acne lesions. It is an effective treatment that improves the appearance of scars. Treatment of post-acne scars will always be performed after active acne lesions have healed.

How is subcision performed and what is the principle?

Subcision is performed on areas where there are atrophic scars. A topical anesthesia (anesthetic cream) is applied, which makes the procedure painless, after which a special needle or cannula will be inserted into the skin. The doctor uses these instruments to break the bands of fibrous tissue that pull the skin down. This fibrous tissue is the cause of deep scars and breaking it will allow the skin to heal properly by generating new collagen fibers.

What are the side effects?

As with any procedure, there are possible side effects and risks associated with subcision, including bruising (ecchymosis), swelling (edema), temporary discomfort, redness (erythema), and, in rare cases, the formation of new scars. These are transient and disappear on their own within 24-48 hours.

How many sessions are needed?

The results of the subcision procedure can vary depending on the severity of the scars and the individual's ability to produce collagen. In some cases, several treatment sessions may be required to achieve optimal results. Subcision can also be combined with other acne scar treatments, such as radiofrequency, laser, chemical peels, PRP, to improve results.

What are the contraindications of the subcision procedure?

Contraindications to subcision include: hypertrophic or keloid type scars, treatment with isotretinoin in the last 6 months, active acne vulgaris (especially in severe forms), active bacterial/viral/fungal skin infections.

What are the benefits of circumcision?

Subcision leads to the release of fibrous tissue from under the atrophic scar, and thus, through healing, will lead to the formation of new collagen. This will lead to cosmetic improvement of the scars. 

Most of the time, the subcision procedure is not performed as a single therapy, but in combination with other effective therapies on this type of scars (fractional CO2 laser, surgical excision of scars, PRP, microneedling, radiofrequency, filling with dermal fillers), thus achieving a combined treatment protocol.
